Overview

This report describes maple syrup urine disease, type II, a subtype of maple syrup urine disease (MSUD); MSUD type II exhibits autosomal recessive inheritance. The human gene implicated in this disease is DBT, which encodes a subunit of the branched-chain alpha-keto acid dehydrogenase complex (BCKD), an inner-mitochondrial enzyme complex involved in the breakdown of the branched-chain amino acids isoleucine, leucine, and valine. There is a single orthologous gene in Drosophila, Dbct, for which an engineered null mutation, RNAi-targeting constructs, alleles caused by insertional mutagenesis, and other genetic reagents have been generated.

The human DBT gene has not been introduced into flies.

Animals homozygous for a null mutation of Dbct survive to the pupal stage; at the larval stage multiple MSUD phenotypes are recapitulated, including aberrant branched-chain amino acid accumulation, developmental defects, poor mobile behavior and disrupted L-glutamate homeostasis. Neural abnormalities in the brain and in the developing eye are observed. Administration of the drug Metformin ameliorates many of the observed phenotypes.

[DIHYDROLIPOAMIDE BRANCHED-CHAIN TRANSACYLASE; DBT](https://omim.org/entry/248610)

Symptoms and phenotype

The major clinical features of maple syrup urine disease are mental and physical retardation, feeding problems, and a maple syrup odor to the urine. The keto acids of the branched-chain amino acids are present in the urine, resulting from a block in oxidative decarboxylation. [from MIM:620699; 2025.03.19]

Genetics

Maple syrup urine disease type II (MSUD2) is caused by homozygous or compound heterozygous mutation in the DBT gene. [from MIM:620699; 2025.03.19]

Cellular phenotype and pathology
Molecular information

The branched-chain alpha-keto acid dehydrogenase complex (BCKD) is an inner-mitochondrial enzyme complex involved in the breakdown of the branched-chain amino acids isoleucine, leucine, and valine. The BCKD complex is thought to be composed of a core of 24 transacylase (E2) subunits, and associated decarboxylase (E1), dehydrogenase (E3), and regulatory subunits. The DBT gene encodes the transacylase (E2) subunit. [Gene Cards, DBT; 2020.10.24]

      Gene Snapshot
Dihydrolipoamide branched chain transacylase E2 (Dbct) encodes the E2 subunit of the mitochondrial branched-chain alpha-ketoacid dehydrogenase complex that catalyzes the oxidative decarboxylation of branched-chain alpha-ketoacids to branched-chain acyl-CoAs. Dbct catalyzes the transfer of the acyl group generated by the E1 subunits, Bckdha and Bckdhb, to CoA. The complex is involved in the second step of L-valine, L-isoleucine and L-leucine catabolism.
